Make a referral.
Families are welcome to self-refer — no doctor's referral is needed. GPs, paediatricians, educators and allied health colleagues can also refer directly.
Referral form
An external referral form can be embedded here, capturing child and family details, funding type, service area and the reason for referral.
What happens next
- 1. We review the referralUsually within two business days.
- 2. We contact the familyTo confirm service area, funding and availability.
- 3. Assessment is scheduledWith written feedback shared back to the referrer where consent is given.
